Action for ME >

Grant value: £32,278

The organisation will undertake a feasibility study to investigate the potential of using Skype and other new technologies to create virtual support groups for those living with M.E. 

Alzheimer Scotland >

Grant value: £28,651

The organisation will develop their work in West Lothian by offering 1:1 self management support in people’s homes in addition to running dementia cafes for those affected by the condition and their carers. 

Angus Cardiac Group >

Grant value: £68,000

The organisation will train exercise leaders in health behaviour change techniques and will investigate and develop new exercise classes such as Wii and 1:1 seated opportunities. These developments will focus on carers and those who are housebound.

The organisation will also work with Exercise Leaders to improve their understanding of self management as a means of influencing change in attendees.

Asthma UK Scotland >

Grant value: £83,377

The organisation will work to mainstream Asthma education in all secondary schools via the Curriculum for Excellence. The organisation will work with Education Scotland to develop an online resource that will enable teachers to deliver Asthma Education across the curriculum.

Castlemilk Stress Centre >

Grant value: £26,100

The organisation will develop a more structured version of their stress management programme.

Clydeside Action on Asbestos >

Grant value: £72,500

The organisation will produce a digital self management programme in partnership with their support group in Ayr which will be distributed to all members and GPs across Scotland.

The organisation will also work in partnership with respiratory Health Care Professionals and those living with the conditions to form two working party groups that will produce key recommendations for self management of asbestosis and create a system which healthcare professionals will use to link people to self management information at the point of diagnosis.

Grant value: £58,258

The organisation will explore a sustainable Recovery Coaching model by training Recovery Coaches as trainers and co-supervisors. The organisation will also work to embed Recovery Coaching in other organisations by training staff supporting those in recovery and by incorporating recovery coaching into circles of care.

Deafblind Scotland >

Grant value: £38,000

Will develop their drama group in 4 ways:

  • Involving those with a single sensory impairment.
  • Involving deafblind people who communicate using only tactile communication methods
  • Create greater understanding of self management by providing awareness training to all involved in the project.
  • Recruiting volunteers to act as communicators and sustain the group beyond the funding period

Diabetes UK >

Grant value: £108,489

The organisation will continue to build the capacity of those living in the South Asian community in Glasgow to self manage. 

Dunoon Link Club >

Grant value: £30,887

The organisation will build the capacity of those living in the area to self manage their mental health using Wellness Recovery Action Planning (WRAP). The organisation will also train participants as WRAP trainers to ensure the sustainability of the service. 

Healthy Valleys >

Grant value: £60,444

The organisation will create an outreach version of the My Way programme by taking it to four satellite locations in rural Lanarkshire where demand is high and services are lacking.

Healthy Valleys will also develop the existing programme in two main ways:

  • Incorporating complementary therapies and listening ear sessions into the existing programme.
  • Creating a volunteer development programme that will work towards the sustainability of the service.

The organisation will establish three hearing loss support groups across Scotland.

The organisation will work in partnership with Alzheimer Scotland to develop their electronic person centred planning tool for those living with dementia, and explore how it could be applied to other long terms conditions. 

Lothian Centre for Inclusive Living (LCIL) >

Grant value: £73,567

The organisation will work to develop a strategy for ensuring the sustainability of the Your Call service and will work towards partnership agreements with Health Boards across Scotland. 

Mental Health Foundation >

Grant value: £58,266

The organisation will take forward findings of original research by developing standards for peer support and by producing and piloting a standard training module for peer support.

Momentum >

Grant value: £39,278

The organisation will develop their Acquired Brain Injury (ABI) self management programme to support young men living with an ABI in the Grampian area. 

No Strings Attached Scotland >

Grant value: £28,195

The organisation will open their successful music self management programme to all young people in the Pririe Park area of Glasgow. The organisation will develop parent peer support roles to encourage and sustain engagement with the work. No Strings Attached will also create a digital music and Asthma resource and will appoint a Development Worker to explore the future sustainability of the project.

Parents of Autistic Spectrum Disorder Adults (PASDA) >

Grant value: £47,900

The organisation will work in partnership with Scottish Autism to develop a newly diagnosed version of their support service. This will include a training sessions for parents of those newly diagnosed and a series of 1:1 sessions that will provide personalised support on specific issues. The organisation will also adapt and develop the complementary therapy sessions offered to members based on learning from the initial project.

Pink Ladies First Scotland >

Grant value: £75,640

The organisation will develop the courses they offer in two ways; by offering a more supported programme of initial engagement and by running an advanced programme for Pink Lady graduates.

The organisation will also investigate how the Pink Ladies courses could be developed to provide tailored courses for specific groups of women. 

PND Support Network >

Grant value: £112,900

The organisation will develop their project in three main ways:

  • By working directly with maternity unit staff, health visitors, baby clinic staff to support pre and post natal women.
  • Develop the website to offer enhanced access to web and forum based support.
  • Create an online information point.

Royal National Institute for the Blind (RNIB) >

Grant value: £18,000

Participants will work in partnership with Young Scot and the Youth Parliament to educate their sighted peers about sight loss. 

Scottish Association for Mental Health (SAMH) >

Grant value: £142,230

The organisation will continue to work with three development sites to offer participants enhanced versions of each programme and further training opportunities as part of their Get Active programme. The organisation will document and share the learning from the new project by producing a DVD of participant journeys.

SKS Scotland >

Grant value: £58,068

The organisation will establish a cooperative for those living with M.E who wish to work on a self-employed basis. The model will provide people the opportunity to work flexibly while being supported to self manage effectively. 

Solar Bear >

Grant value: £35,000

The organisation will use theatre skills to teach participants self management tips and techniques based around themes of advocacy mentoring and leadership. The organisation will also train past participants as project advocates.

Speak Easy >

Grant value: £29,237

The organisation will hire an Information and Support Worker who will undertake various activities to develop and expand work around self management. The worker will focus on engaging those who are newly diagnosed and also families and carers of those living with the conditions. 

Tagsa Uibhist >

Grant value: £9,000

The organisation has previously worked on a one to one basis supporting those on the islands living with mental health conditions. The organisation will now establish support groups in the main towns of the three larger islands. In addition Tagsa Ubhist will pilot outreach groups in the very remote areas to find out if it is better for these communities to access peer support where they are or to join groups in the larger villages. 

Target Ovarian Cancer >

Grant value: £12,785

The organisation will undertake an action research project to investigate and explore the potential for a Scottish network for women living Ovarian Cancer. The organisation will use events to consult with women across Scotland to find out their self management needs and then incorporate these into their future planning for a Scottish Ovarian Cancer network.

The Haven >

Grant value: £68,068

The organisation will:

  • Recruit and train volunteers to co-facilitate the lymphedema self management course.
  •  Run a male only self management group
  •  Run a 1:1 version of the course for men and women who, because of the nature of the condition, do not feel comfortable in a group setting.
  •  Expand the reach of the programme to geographically isolated areas where provision is lacking and transport is difficult

The Thistle Foundation >

Grant value: £150,000

The organisation would like to catalyse the culture shift in how services are offered across sectors and how services could be delivered to suit the needs of those living with long term conditions.

The organisation will continue to work in partnership with NHS Lothian and NHS Mid Lothian to deliver the project and will focus work on Social Care.

Waverley Care >

Grant value: £85,872

The organisation will design and deliver three new Life Coaching Programmes; two targeting prisoners living with HIV and/or HCV and the other targeting the South Asian community living with HCV.

Waverley care will also design and deliver a new training programme to 14 frontline staff focusing on the delivery of 1:1 Life Coaching interventions. The organisation will train Peer Mentors, to support the delivery of the Life Coaching Programmes and the development of Peer Support Groups in all locations where the service is delivered.
